Do I need a referral from my primary care doctor?▾
No. You can call us directly to schedule a screening. For Medicaid HMO plans, we will verify whether a prior authorization is required and help you through that process.
Can I pay out of pocket? What does self-pay cost?▾
Yes, we accept self-pay (cash or card). Qualifying patients may receive a sliding-scale fee based on income. Call admissions at 1 (888) 975-3422 Option #3 for current rates.
My plan isn't listed here — what do I do?▾
Call us. We frequently update our network and may be able to accept your plan or help you find a solution.
Is there a co-pay?▾
Co-pays depend on your specific plan. Our billing team will verify benefits and walk you through any out-of-pocket costs before your first visit.
