Admissions Screening & Intake

At Legacy, we are committed to providing clients with the highest quality care starting from the very first step. Our Admissions Department features a compassionate team of licensed screening assistants, including bilingual staff fluent in English and Spanish, ready to guide you through the intake, screening, and assessment process.

What to Expect

Once you’re referred to Legacy, an admissions assistant will reach out to gather necessary information and answer any questions you may have. An intake appointment will be scheduled at your convenience. During this visit, you’ll complete a screening and may be referred for services such as:

  • Psychological or Psychiatric Evaluation
  •  Medication Management
  • Therapy
  • Targeted Case Management

You may also contact us directly if you’re seeking services without a referral.

Eligibility

We serve adults and children age 5 and older. To be eligible for services, you must:

  • Have insurance accepted by Legacy (see our Accepted Insurance list)
  • OR, if uninsured, complete a financial assessment to determine eligibility for discounted rates via our sliding fee scale
  • Self-pay is also accepted

Note: We do not accept private insurance.

What to Bring to Your Screening

Please bring the following documentation to your screening appointment:

  • Valid Driver’s License or photo ID
  • Insurance Card (if applicable)
  • Legal guardianship documentation (for minors)
  • Birth Certificate
  • Court Orders
  • Any other documents as requested

Referral Form

We offer multiple ways to submit a referral:

  • By Phone: Call Admissions at 772-873-8811 (Choose Option #3 for new or returning patients)
  • By Fax: Complete and fax the referral form to 772-232-6307
  • By Email: Fill out, scan, and send the form to admissions@legacybhc.com
